How you can give us feedback:

  • Speak directly to your worker or their Service Manager
  • You can also give feedback using our Comments and Complaints form.

If you do not feel confident approaching SMART about a complaint or feel you need help in putting your comments forward, you can ask to speak to your local advocacy service.
