About the SMART Specialist Prescribing service

SMART is the single point of contact for all alcohol referrals in Bracknell, Maidenhead, and Windsor.  We can treat anyone with opiate or alcohol problems and provide substitute prescribing.   The aim of treatment can be stabilisation or detoxification.   Services are provided by a multidisciplinary team including a nurse and substance misuse workers who liaise with partner agencies as required and work in partnership with prescribing doctors.  Support includes:

  • triage
  • stabilisation
  • ongoing support
  • key working
  • one-to-one care planning
  • harm reduction advice
  • onward referrals into other services.

About our Alcohol Service

Who is this service for?

The service is aimed at anyone aged 18 or over who is worried about their drinking.   SMART can take referrals from GPs, other agencies and self referrals from individuals.    

Service in detail

Triage appointment

All service users are given a triage appointment at the earliest opportunity.  This appointment determines the priority needs of the service user.  The service user is assessed to determine the treatment required.  This includes community-based detox; in-patient detox; a reduction programme or other appropriate support..

Care coordination

We work in close partnership with service users and partner agencies to ensure that a comprehensive care plan is produced for each service user.    The care plan identifies the issues to be addressed and establishes clear, achievable goals that the service user and worker can work towards.  Typically they will also offer advice and support around:

  • Reduction programme
  • Detoxification
  • Structured intervention
  • Harm reduction
  • Relapse prevention
  • Onward referrals such as: mental health; housing; counselling; family support or for any other identified need. 

Alcohol Support Groups

We also run weekly alcohol groups in Bracknell and Maidenhead. These give advice and support on staying abstinent. 

About our Substitute Prescribing

Who is this service for?

The service is aimed at anyone aged 18 and over who is using opiates,  requires specialist support and needs to be seen on a regular basis.  They will be either:

  • Dual-diagnosis (with long and enduring mental health problems);
  • Injecting unsafely or
  • Pregnant

and therefore in need of intensive support.

We take referrals from GPs and other agencies.  If service users wish to self-refer, they can access treatment through Open Access services (Run by SMART).

Service in detail

Triage

All service users are given a triage appointment at the earliest opportunity.  This appointment determines the priority need of the service use..  The service user is assessed as to the treatment required and an appointment will be made at a clinic with a prescribing doctor for Titration (Determining the required dosage of substitute medication)

Care planning

Following stabilisation, staff will work with the service user to develop an integrated treatment and care plan.  The care plan identifies clear, achievable goals that the service user and worker can work towards.  Typically they will also offer advice and support around:

  • Reduction programme
  • Detoxification
  • Structured intervention
  • Harm reduction
  • Relapse prevention
  • Onward referrals such as: mental health; housing; counselling; family support or for any other identified need.
